While some states have regulations that mandate certified athletic trainers (ATCs) at the high school level, many are in violation. For example, the state of Rhode Island has a state law that an ATC must be at athletic events. However, according to Dr. John P. Sullivan, University of Rhode Island sport psychologist, most schools are in violation (Smith, C. [link widoczny dla zalogowanych], 2010).
High School Athletic Departments Understaffed for Adequate and Appropriate Medical Care
High schools try to provide medical coverage for their athletes by using either full-time or part-time ATCs, coaches with first aid training, and emergency medical technicians for game coverage (football). The rationale is that if an ATC is not available, the coach can provide emergency care until professional help arrives.
Although this sounds good in theory [link widoczny dla zalogowanych], the reality is that coaches are ill-prepared to handle emergency medical situations. Compared to a four year Bachelor of Science degree in Athletic Training, an eight hour first aid course is not going to adequately prepare a coach to handle a catastrophic medical and/or orthopedic injury.
Sudden death in athletes can be caused by several different physiological factors including congenital heart disease, brain injury, and heat illness. While some sudden deaths may not be preventable, others can be prevented with the right medical professionals working alongside the athletes.
At least three high school football players have died this past fall while competing in the sport of football (Smith, C., 2010). Last month, high school quarterback Reggie Garrett collapsed on the sideline during a Friday night football game just after throwing for a touchdown.

In many high schools with part-time ATCs, the ATCs attention is usually focused on providing care for the higher visibility sports such as football and basketball. If one was to look at the medical coverage by ATCs of football at the college level, it would be evident that athletic departments hire multiple ATCs just to cover football (4-6). Larger Division I programs typically have more ATCs if certified athletic training graduate assistants are included in the coverage.

Compounding the problem is that many high school athletic departments who do choose to hire ATCs only have part-time positions or may only have one ATC treating all of their athletes and providing medical coverage for all of their sports. In other words, most high school athletic departments are not prepared to provide adequate medical care for all of their athletes during practices and events.
Professional sports and collegiate athletic departments would not begin a practice or start an athletic event without the availability of appropriate medical care on the sidelines. While other allied health professionals are helpful, the certified athletic trainer is the only allied health professional academically prepared specifically to prevent, manage, and treat emergent and non-emergent sports injuries.
A report from D. Shabner (September 18, 2010) stated that coaches and team doctors tried to help Garrett. He was pronounced dead at the hospital later that evening. The results of an autopsy will not be known for several weeks.

It is an interesting phenomenon that as the skill, competition, and fitness levels of athletes improves (college and professional level) [link widoczny dla zalogowanych], the medical care provided for them also improves. The irony is that when young athletes need non-emergent treatment and emergency medical care the most, it is rarely available.
